We require all new patients to fill out new patient paperwork at their first visit. We offer the opportunity to fill the forms out ahead of time. You can find them below. Each patient will need to fill out 4 forms: New Patient Information, Financial Agreement, Insurance Authorization Form, and Patient Privacy Agreement.
Treatment of a Minor – ONLY needed for patients UNDER 18 years old.
**PLEASE NOTE: If you are under the age of 18 we require a parent/guardian to be present at each visit. In addition, please fill out the Treatment of a Minor form.
